How to Request a Transcript

Complete a transcript request form. Include the name you used during your attendance at JTS, your JTS student ID number or Social Security number, the dates you attended JTS, whether you graduated or withdrew, which JTS school you attended, and the address(es) where you would like the transcript(s) sent. Mail or hand-deliver the completed, signed request form (original signature required—fax and email copies not accepted) with a check or money order for the appropriate fees* to the following address:

* The official transcript fee is five dollars for the first copy and two dollars for each additional official copy. Unofficial student copies and JTS interoffice copies are free.  


The Registrar's Office provides services related to courses, registration, grades, and transcripts. Use the links on this page to plan your schedule, see important deadlines listed on the academic calendar, and download transcript request forms, drop/add forms, independent study forms, incomplete forms, and consortium registration forms. The JTS Registrar's Office is located on the first floor of the Schiff building. Staff members are available for walk-in service Monday through Thursday, 10:00 a.m.–3:00 p.m., and Friday, 10:00 a.m.–2:00 p.m. Staff members are available at other times by appointment.
